Adding a dynamic form

To add new dynamic form, take the following steps:

  1. Log in to your TRAX account.
  2. Go to Dynamic Forms then select the form from the list where you want to add a new form.



  3. Click Add New Form.



  4. Select Evaluatee and Interaction Date.



  5. Tick applicable reporting indicator(s).


    • Approval Required - select this if the form needs to be approved by the Team Lead first.
    • Nesting Period Required - select when the agent has recently started taking phone calls and is still in the consolidation period
    • For Calibration - select when an evaluator evaluates a call that has already been assessed.
    • Self Evaluation - select this if you are making a self-evaluation form.
    • High-importance - select if the form should be prioritised.

  6. Click Save and Send To Assigned.



    The form is now assigned and will now appear in the open forms. 

     

    Note: The evaluatee/assignee will receive an email notification about the form.
